Registered Nurse - Transitional Care

Location: Portland, ME

Assignment Duration: This is a 13-week contract position.

Schedule: Nights: 7:00 PM - 7:30 AM, with every other weekend required.

Job Description: The Registered Nurse (RN) will provide high-quality patient-centered care in a Transitional Care setting with a focus on LTC/SNF. The RN will be responsible for managing patient care, collaborating with the healthcare team, and utilizing electronic medical records (EMR) for effective charting and documentation.

Responsibilities:

• Provide nursing care to a diverse patient population.
• Maintain patient documentation accurately in EMR.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Monitor and assess patient conditions and progress.
• Administer medications and treatments as prescribed.

Required Experience / Certifications / Licensure:

• Must have recent acute care experience.
• Experience with EMR charting is preferred.
• Current BLS certification is required.
• Active RN license in the state of ME.
