Manage chronic disease and provide symptom management, goals-of-care discussions, and advanced care planning. Provide care with a mix of 75% inpatient and 25% outpatient duties during weekdays. Perform inpatient rounding for hospice patients (up to 6 patients per day). Rotate on-call duties for Hospitalists, admit patients, and manage open ICU admissions.
