Minimum Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona Physical Therapist license required for submission or proof of application with an estimated time of approval.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in skilled nursing, long-term care, or post-acute rehabilitation settings preferred.
• Ability to provide assessment, treatment planning, and skilled therapeutic interventions for adult and geriatric populations.
• Client requires availability to work five days per week and a 32-hour guaranteed workweek as noted in order details.

Preferred Qualifications

• Experience with mobility training, fall prevention, and adaptive equipment in SNF settings.
• Prior travel therapy experience preferred.
• Experience with electronic medical records and documentation in skilled nursing facilities preferred.

Assignment Details

• Shift pattern: Five days per week, 32-hour guarantee.
• Arizona license required for submission or must have applied with an ETA for license approval.
• May be asked to float to other nearby facilities as operational needs require.
