Outpatient Physical Therapist

SUMMARY: St. Andrews is hiring a full time Outpatient Physical Therapist . The primary purpose of the position is to ensure the highest quality of resident care available, support staff and establish a positive reputation in the community while delivering on the Eduro HealthCare Values of wellness, compassion, customer experience and company results. This is a dynamic, fun, and growing team.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:Adherence to PoliciesMust show courtesy and respect to other employees, residents, family members, visitors, government agencies, the general public, etc.Abide by Eduro HealthCares drug and alcohol policy.Maintain good personal hygiene and dress and groom appropriately.Report on time as scheduled and follow all company policies and procedures.Attend staff meetings and in-service sessions.Become thoroughly familiar with emergency procedures.Must be able to perform duties in a timely fashion, and within the prescribed sequences and schedules.Must be cooperative with other departments and be courteous and respectful in dealing with them at all times.Participate in and respond professionally to surveys (inspections) conducted by government agencies.Physical Therapist Duties includeEffectively screens/evaluates/rehabiltates patients with physical functioning disorders and/or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.Utilizes standardized assessments and evidence-based practice to support clinical interventions.Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.Assesses patients progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.Accurately records treatment minutes for services provided per all regulatory and clinical requirements.Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.Participate in facility and company required trainings, in-services and conferences.Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements.Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.Adherence to APTA and State Required practice acts, code of conduct, ethics during professional practice as a Physical Therapist.Patient CareBe personally involved with all aspects of resident care to the extent necessary to assess its quality.Respect all resident rights, including the confidentiality of resident care information.Report immediately to the proper legal authorities if you have reason to believe a resident has been physically, emotionally, or sexually abused, or been a victim of theft of their personal property.Create and maintain an atmosphere of warmth, cheerfulness, enthusiasm, and love, giving the resident the quality of service you would want to receive personally.
