Employment Details Location: San Antonio, TX 78216 Duration: 13 weeks Shift: 12‑hour days, 36 hours per week (6 days) Employment type: Travel Physical Therapist – Skilled Nursing / Long Term Care (Contract) Job Summary Seeking a dedicated Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing and Long-Term Care setting. The PT will evaluate patients, develop individualized treatment plans, and deliver therapy focused on improving mobility, strength, and functional independence. Key Responsibilities Evaluate patients and create individualized treatment plans Provide therapeutic exercise, gait, balance, and mobility training Monitor patient progress and modify treatment plans as needed Educate patients and caregivers on therapy techniques and safety Maintain accurate and timely documentation Collaborate with interdisciplinary rehab team Promote optimal recovery and patient outcomes Qualifications Must have an active Texas Physical Therapist license Strong patient assessment and treatment planning skills Experience in Skilled Nursing / Long-Term Care settings Knowledge of therapeutic exercises and mobility training Excellent communication and teamwork skills Minimum 3 years of PT experience (SNF / Long-Term Care experience preferred) Compensation Pay package is based on 12‑hour shifts and 36 hours per week (subject to confirmation) with a tax‑free stipend to be determined.
