We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full‑Time Physical Therapist (PT) in Hayward. Position Overview As a Physical Therapist (PT) you will evaluate patients’ health, create and implement care plans, and track their progress to maintain the highest standards of care. You will work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field. Responsibilities Provide, coordinate, and oversee skilled intermittent care to patients in their homes and instruct patients and caregivers to promote maximal functional status. Regularly evaluate and re‑evaluate patient status. Administer treatments, monitor patient conditions, and report changes in condition to Physician, Case Manager, and Director of Nursing as needed. Initiate, develop, implement, and revise the patient’s plan of care (POC). Address all problems identified in the POC. Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs related to the patient’s condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements Current PT license to practice in California (required). At least 1 year of experience in home health or hospice care (required). Current California driver’s license. Ability to travel within a 15–25 mile radius on a regular basis.
