Location: Pennsylvania (Home Health – Lebanon & Surrounding Areas)

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

On‑Call/Weekends: None

Travel: ~20% local travel to patient homes

Overview

Join our client’s compassionate care team as a Home Health Physical Therapy Assistant , supporting patients directly in their homes throughout Lebanon, PA, and nearby communities. This role offers a supportive environment with mentorship, flexible scheduling, and opportunities for professional growth. You will play a key role in delivering and managing patient care plans while collaborating closely with a dedicated clinical team.

Key Responsibilities

• Collaborate with the clinical team to support the management and delivery of patient care plans
  
• Provide care to patients in Lebanon, PA and surrounding areas
  
• Complete charting and documentation within required deadlines
  
• Self‑manage daily schedule, travel, and patient visits effectively
  
• Maintain professionalism, communication, and patient‑centered care at all times
  

Travel Expectations

• Ability to spend approximately 20% of the workday driving to and from patient homes
  

Schedule

• Monday – Friday, 8:00 AM – 5:00 PM
  
• No on‑call, weekends, or holidays
  

Requirements

Required Qualifications

• Associate degree or higher
  
• 1+ year of experience in ANY of the following:
  
• Home Health Care
  
• Home Health
  
• 1+ year of experience in ANY of the following:
  
• Electronic Medical Record (EMR) systems
  
• Practice Partner EMR
  
• EHR Implementation
  
• Valid Pennsylvania Driver’s License
  
• Active Physical Therapy Assistant (PTA) License in Pennsylvania
  
• Has not applied to or worked for Residential Healthcare Group within the last 6 months
  
• Comfortable with required travel for the position
