Physical Therapist - VNS in Westchester

Responsibilities:

• Assessment : Evaluate patients for functional limitations and collaborate with patients, families, nurses, physicians, and other health team members to develop a personalized therapy plan.
• Direct Care : Provide therapy services to treat medical and surgical conditions using accepted physical therapy modalities.
• Education : Instruct and supervise patients, families, home health aides, and others on home exercise programs and aspects of patient care.
• Ongoing Evaluation : Continuously assess the patient's condition and adjust the therapy plan as needed.
• Communication : Report significant clinical information to physicians, clinical managers, and other team members. Document clinical evaluations, treatments, and patient responses to therapy.

Qualifications:

• NY Professional License and current registration as a Physical Therapist.
• Minimum 1 year of full-time experience providing physical therapy services.
• Ability to assess and respond to patient and family needs in a home care setting.
• Home care experience is preferred but not required.
• Ability to use technology (such as tablets) for patient documentation and care.
• Experience with Oasis assessments is a plus.
• A valid driver's license and access to a reliable automobile for travel in the field.

Salary:

• $95,000 to $108,000 annually , based on experience.
