We’re Hiring: We are seeking a full‑time Nurse Practitioner or Physician Assistant experienced in Skilled Nursing Facilities to join our team as a Regional Lead. This role combines clinical practice, regional oversight, and business development, responsible for ensuring clinical excellence and growth. The ideal candidate will be a strong clinician, mentor, and relationship builder in skilled nursing. Day‑to‑Day Responsibilities Primarily responsible for regional oversight of clinical care and facility relationships, you will also round 2‑3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations. Clinical point of contact for all Providers in your region Clinical training to ensure all Providers display excellent clinical decision making and bedside manner Time shadowing each Provider in the region Regular clinical and facility meetings with facility staff Focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery. Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs. Supporting physiatrists on‑site and operating with a high level of autonomy Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry This role is best suited for a self‑motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes. Qualifications: Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA). DEA licensed or willing to obtain DEA licensure Nursing home and/or physiatry experience Comfort and interest in performing peripheral joint injections (knees, shoulders, hips). A genuine passion for providing professional, personalized care in an autonomous environment Strong communication skills, with the ability to collaborate with multidisciplinary teams Familiarity with EHR charting Experience in a clinical leadership or supervisory role Willingness to travel regularly within the assigned region Why us?
