TravelNurseSource is working with Pyramid Consulting, Inc. to find a qualified Hospice RN in Grand Junction, Colorado, 81501!

Job Responsibilities – Registered Nurse (RN) – Hospice

• Provide compassionate, patient-centered nursing care to hospice patients with terminal illnesses, focusing on comfort, dignity, and quality of life.

• Assess patients’ physical, emotional, and psychosocial needs and develop individualized plans of care in collaboration with the interdisciplinary hospice team.

• Monitor patients’ conditions, evaluate symptoms, and implement appropriate interventions to manage pain, shortness of breath, nausea, anxiety, and other end-of-life symptoms.

• Administer medications and treatments as prescribed, ensuring safe medication management and effective symptom control.

• Conduct regular patient assessments and document vital signs, clinical observations, changes in condition, and responses to treatment.

• Educate patients and family members about disease progression, medication administration, symptom management, and end-of-life care.

• Provide emotional support, counseling, and guidance to patients and families throughout the hospice care journey.

• Coordinate care with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.

• Communicate changes in patients’ conditions to the appropriate healthcare providers and implement updates to the plan of care.

• Maintain accurate clinical documentation in accordance with hospice policies, regulatory requirements, and established nursing standards.

• Support family members and caregivers with bereavement resources and guidance as appropriate.

• Ensure compliance with infection control practices, patient safety standards, and hospice care regulations.

Requirements

Required Qualifications

• Current and valid Registered Nurse (RN) license in the state of practice.

• Previous nursing experience in hospice, palliative care, home health, or a related clinical setting preferred.

• Strong knowledge of pain management, symptom control, and end-of-life care principles.

• Excellent communication, assessment, care coordination, and documentation skills.

• Ability to provide compassionate care while supporting patients and families during end-of-life transitions.
