• Advanced practice provider (PA/NP) opportunity with an established senior health clinic

• Full-time role providing comprehensive care to older adults in clinic and affiliated nursing home settings

• Monday"Friday schedule with no weekend or holiday requirements, supporting strong work"life balance

• Responsibilities include geriatric assessments, chronic disease management, coordination of care, and collaboration with a multidisciplinary team focused on quality of life for aging adults

• Licensed or license-eligible physician assistant or nurse practitioner required

• Minimum 1"2 years of experience in geriatrics, nursing homes, or long-term care preferred
