Physical Therapist

Territory: Brighton, Lyon Charter Township, West Bloomfield Charter Township, Farmington Hills, Novi, White Lake Charter Township, Wixom, Milford Charter Township. Schedule: Mon‑Fri 8am‑5pm / on call 2 weekend days a month. Agency: Beaumont Home Health / Hospice. Sign on Bonus: $15,000.00. Job Summary The Physical Therapist I (PT) provides comprehensive skilled services for patients in their homes as ordered by the attending physician. The PT educates patients and their families, ensures patient safety, maintains accurate documentation, and remains compliant with all legal regulations. Key Responsibilities Provide skilled physical therapy in a patient’s home per the physician’s order. Observe, record, and report patient reactions, changes, and progress to the attending physician and agency staff. Perform OASIS assessments, develop individualized plans of care, and adjust as needed. Develop plans of care and treat patients to relieve pain, restore function, and maintain maximal performance using physical means such as exercise, massage, heat, water, etc. Arrange provision of services not deliverable in the home on an outpatient basis. Promote continuity of care with appropriate admissions, transfers, and discharges. Counsel patients and families regarding disease/injury and management throughout all phases of therapy. Incorporate patients and families in the development of the care plan. Instruct on proper use of equipment (wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices). Ensure patients/families understand financial responsibilities for needed equipment using the DME department or MSW. Teach preventative measures to reduce re‑hospitalizations and support informed decision‑making. Oversee and supervise therapy services performed by PTAs and nurse assistants when appropriate. Evaluate the plan of care based on patient response and medical condition; adjust as needed. Communicate supervisory visit results with PTAs and home‑health aides. Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Conduct chart audits at the agency site as requested. Maintain an active PT license in the state of employment. Communicate with the scheduler about any changes needed. Participate in on‑call requirements and case conferences. Attend in‑service trainings and mandatory agency meetings. Participate in orientation/training of new employees. Maintain a professional appearance as a representative of the company. Understand and practice agency policies and procedures. Drive to the patient’s primary location per scheduled visit. Document all aspects of subsequent, discharge, eval/recert/resumption visits within 24 hours. Complete and submit all required documentation within specified company requirements. Establish and follow the plan of care as permitted within the scope of practice for a Licensed Physical Therapist, per state regulations. Perform other duties as assigned. Qualifications Minimum of one year of experience as a PT in an acute setting; home care experience desired. Effective communication skills and ability to create positive impressions with patients, families, physicians, and coworkers. Knowledge of therapy best practices. Ability to make appropriate physical therapy judgments. Ability to identify situations and handle them with optimal solutions. Detail‑oriented and observant. Disciplined work ethic with ability to prioritize and be timely. Valid driver’s license and auto insurance with your name listed as a driver. Education and Credentials Physical Therapist with a current license in the state of employment.
