Collaborative Chronic Disease Management: Partners with providers and the care team to actively monitor chronic disease and prevention reports, initiating timely follow‑ups or referrals to optimize patient health and well‑being. Communication Powerhouse: Seamlessly manages multiple communication channels (phone, email, in‑person), ensuring clear and effective information flow across the care team and with patients, in both Spanish and English. Organization & Efficiency Expert: Masters both electronic and paper systems, expertly handling inboxes, scheduling, and follow‑ups to keep the team running smoothly and patients well‑informed. Patient‑Centric Advocate: Proactively participates in reminder calls, follows up on no‑shows, and ensures patients receive critical information about referrals, treatments, and medications. Detail‑Oriented Documenter: Maintains accurate and comprehensive patient records, utilizing SOAP notes, EPIC phrases, and clear communication to support high‑quality care and collaboration. Provider / Team Support Dynamic Clinical Support: Provides comprehensive clinical assistance to providers, performing a wide range of tasks from patient intake and vitals to administering medications and assisting with procedures. Qualifications Minimum Qualifications. Associate’s or Bachelor’s degree in nursing from an accredited nursing program. Licensure: valid license to practice as a Licensed Practical Nurse in the State of Oregon by the time of hire. Minimum one (1) year of nursing experience. BLS/CPR certification required. Spanish / English languages (preferred). Preferred Qualifications. Experience providing services to diverse and vulnerable communities. Demonstrated experience in primary care.
