We are seeking an adaptable and highly collaborative Occupational Therapist (OT) for a full-time contract position in Elk Grove. In this dual clinical and management role, you will coordinate and deliver vital restorative and rehabilitative occupational therapy services to seniors both at our day health center and in their homes or residential care facilities. You will partner closely with staff Physicians, Physical Therapists, and an interdisciplinary team (IDT) to optimize participant safety and performance of Activities of Daily Living (ADLs). Additionally, you will serve as a key member of our management team, directly supervising and mentoring Therapy Assistants and guiding them in executing Functional Maintenance Programs (FMPs). Duties & Responsibilities Clinical Assessment & Therapy Delivery
- Comprehensive Assessments: Conduct occupational therapy evaluations to identify participants' therapeutic needs, preferences, and goals, actively contributing to IDT meetings to formulate individualized care plans.
- Therapeutic Interventions: Deliver and document physician-ordered occupational therapy interventions. These include activities designed to improve muscle re-education, perceptual-motor skills, cognitive function, balance, and endurance.
- Home & Facility Visits: Conduct local mobile visits (typically 4-6 patients per day) to perform safety assessments and treatment sessions within participants' homes or Skilled Nursing/Assisted Living Facilities (SNFs/ALFs).
- Adaptive Equipment Management: Order, track, and manage routine maintenance for therapy equipment and adaptive devices.
- Specialized Care Coordination: Support smooth care transitions between hospitals, SNFs, and homes. Participate in end-of-life care coordination and assist marketing teams with prospective participant enrollment.
- Clinical Supervision: Direct, train, and instruct Therapy Assistants in executing Functional Maintenance Programs (FMPs).
- Mentorship & Team Leadership: Manage, supervise, and mentor direct reports, utilizing operational data and reports to prioritize workloads and resolve performance barriers.
- Caregiver Education: Provide specialized, hands-on training and instruction to staff, participants, and family caregivers in therapy techniques.
- Other duties as assigned.
- Education: Bachelor's Degree in Occupational Therapy from an accredited program matching AOTA and ACOTE standards.
- Certification: Active initial registration with the National Board for Certification in Occupational Therapy (NBCOT).
- Licensure: Valid license (or immediate licensure eligibility) to practice as an Occupational Therapist in the State of California.
- Life Support: Active Basic Life Support (BLS) certification.
- Clinical Experience: Minimum of two (2) years of Occupational Therapy experience in a clinical setting working with a frail or elderly population.
- Supervisory Experience: Minimum of two (2) years of direct supervisory or management experience, with a proven ability to mentor, train, and develop clinical team members.
- Mobility: Must possess a valid driver's license and reliable personal transportation to complete required field-based mobile therapy visits.
- Work Authorization: Must be legally authorized to practice and work in the State of California.
- Data-Driven Management: Prior experience working in a data-driven healthcare organization, leveraging reporting metrics to manage teams and prioritize assignments.
- Adaptability: Exceptional communication and organizational skills, with a proven ability to work independently, manage shifting priorities, and collaborate with diverse cultural groups.
- Work Type: Full-Time Contract Position (40 hours per week).
- Work Setting: Hybrid clinic-facing and field-based setting. Based out of the center in Elk Grove, CAwith required local mobile travel to patient residences, SNFs, or ALFs.
- Schedule: Monday through Friday, 8:00 AM - 4:30 PM
- Pay: $55-60/HR depending on years of experience.
