RN - Home Health

WCS Health is seeking a Home Health RN in Hicksville, NY to provide, coordinate, and direct skilled nursing care in the patient's home according to physician orders and agency policies. This role requires strong clinical assessment skills, independence, and the ability to manage a diverse caseload that includes Start of Care visits, wound care, follow-ups, and complex home-based interventions. The RN will supervise Home Health Aides and LPNs/LVNs, ensure timely documentation, and deliver high-quality, patient-centered care across a wide travel radius.

Job Function :

• Day Shift.
• 9am -5pm.
• Perform Start of Care, Resumption of Care, Recertification, Transfers, Wound Care, and Routine Visits
• Conduct admission assessments, clinical documentation, and case management
• Provide wound care, pressure ulcer management, drain care, and wound VAC management
• Manage IV therapy, central lines, ports, and peripheral IVs
• Perform phlebotomy, catheter care, ostomy care, and tube feeding management
• Complete medication assessments, pain management, and symptom monitoring
• Educate patients and families on disease processes, safety, and home-based care
• Coordinate interdisciplinary services and follow agency protocols
• Complete all documentation within 24 hours of each patient visit (required)
• Travel between patient homes daily (250-300 miles/week on average)

Job Requirements:

• Active RN License (no pending licenses accepted)
• AHA BLS
• Minimum 1 full year Adult Home Health OR 1 full year Hospice experience (must be recent)
• Must be proficient in OASIS and have used it within the last 6 months
• Must be proficient in HomeCare HomeBase (HCHB) within the last 6 months
• Must have reliable transportation, valid driver's license, and auto insurance
• Must be willing to travel up to 75 miles from home clinic
• Mileage reimbursed at IRS standard rate
• Paid mileage from patient home to patient home
• Commute to first patient and from last patient is not billable
