PRN Case Manager - Inpatient Rehab & Discharge Planning
Memorial Physician Practices
Baptist Memorial Rehabilitation Hospital in Germantown, TN is seeking a PRN Case Manager to coordinate inpatient rehabilitation care, discharge planning and care transitions. You will liaison with patients, families and the health care team to ensure timely, safe transitions and documentation in IPoC per CMS guidelines. Applicants should hold RN or social work licensure and a case management certification; 2+ years in inpatient settings is preferred. #J-18808-Ljbffr Memorial Physician Practices
