About the Role: As a RN MDS Coordinator with Stellar Health Group, you will play a critical role in ensuring the highest quality care for our patients. You will be responsible for coordinating the development and implementation of the Minimum Data Set (MDS) assessments, as well as conducting skilled care assessments and developing care plans. You will work closely with our interdisciplinary team to provide comprehensive care to our patients, and will be responsible for maintaining accurate and timely documentation of patient information.
