Job Description
Title: Licensed Practical Nurse - Geriatrics

Schedule: Full Time, Days and Nights (5x8), Every Other Weekend and Holiday Required

Assignment Details:
- Contract Length: 14 weeks
- Guaranteed Hours: 40 per week

Requirements:
- NY LPN Licensure
- BLS Certification (AHA or RQI)
- 1 year of experience as an LPN, preferably in a long-term care facility

Roles & Responsibilities:
- Provide direct and indirect skilled nursing care to assigned residents, following defined policies and procedures under the supervision of a Registered Nurse.
- Assess and collect ongoing data relevant to residents' physical and psychosocial conditions, communicating findings to the nursing team.
- Collaborate with the Interdisciplinary Team to develop and implement age-appropriate and culturally relevant resident care plans.
- Revise care plans based on resident outcomes and facilitate necessary referrals for comprehensive care.
- Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
- Participate in the mentoring and orientation of new staff, promoting a safe and healthy environment.
