Duties

Provides professional comprehensive community/Public Health nursing services to individuals, families, and community groups using the nursing process to assess, identify outcomes, plan, implement, and evaluate care in accordance with established policies, procedures and standing orders.

Conduct and contribute to community and population health assessments to identify health trends, risk factors, and priority health issues, including social and environmental conditions that affect health.

Plans and delivers health education and prevention activities for individuals, families and community groups to promote wellness and reduce health risks.

Collaborate with interdisciplinary health care teams, schools, community organizations and local, state and federal partners to coordinate care and public health interventions. Participates in the planning and evaluation of public health nursing programs and services to assess effectiveness and support quality improvement.
