Telehealth Family/Adult NP - Chronic Care Clinician

Desort is seeking a Family/Adult Nurse Practitioner to deliver virtual primary and urgent care for adults receiving home and community services. The role focuses on chronic and complex conditions, with telehealth visits, individualized care plans, and coordination with physicians and families.
