RN Home HealthPride Health is hiring a RN Home Health to support our client's healthcare facility in Plains, Pennsylvania.Job Details: Schedule: Monday through Friday, 8:00 AM to 4:30 PM (5 days/week) Location: Plains, PA Float / Coverage Area: Luzerne County and parts of Lackawanna County Job Type: Travel Contract Contract Length: 13 Weeks Pay Range: $50.00 to $53.00/hour Mileage Reimbursement Rate (if applicable): $0.50/ mile *Pay offered is based on experience, expertise, credentialing, and education.Duties:Complete OASIS assessments including Start of Care (SOC), Resumption of Care (ROC), Recertifications, Evaluations, and Discharges.Provide skilled nursing care, including wound care, Foley catheter management, and IV therapy.Educate patients and caregivers on treatment plans, medications, and chronic disease management.Coordinate care and maintain accurate clinical documentation in accordance with agency standards.Participate in on-call rotation, including 1-2 nights per week and 1 weekend per month.Requirements:Education: Graduate of an accredited School of NursingLicense(s): Active Registered Nurse (RN) LicenseCertification(s): Current CPR/BLS CertificationYears of experience: Previous Home Health experience preferredSkills/Specialties/Technology:OASIS DocumentationHome Health Skilled NursingWound CareChronic Disease ManagementFoley Catheter CareIV TherapyPatient EducationCare Coordination
