Home Health Registered Nurse (RN) – Team 4 Job Title: Home Health Registered Nurse (RN) – Team 4 Location: Paramus, NJ Shift: Day Shift | 5x8s (08:00–16:30) Start Date: 01/05/2026 Base Pay Range $65.00/hr - $75.00/hr Job Summary The Home Health RN provides skilled nursing care to patients in their homes, coordinating and delivering patient-centered care in collaboration with an interdisciplinary team. This role focuses on assessment, care planning, medication management, patient education, and accurate clinical documentation. Key Responsibilities Perform comprehensive patient assessments and develop individualized plans of care Deliver skilled nursing services, including medication administration and patient education Document care accurately using the agency’s charting system Coordinate care with physicians and interdisciplinary team members Ensure compliance with infection control, safety, and regulatory standards Requirements Active New Jersey or Compact RN license (in hand at submission) COVID-19 vaccination with booster (no exemptions) Recent supervisory/manager reference (within 1 year) Current Nursys verification required Ability to pass EKG and medication exams (if applicable) Prior home health or travel experience preferred.
