Social Worker (LCSW), Variable Part-Time Day Shift, Maternal Care Center

Shady Grove Medical Center As a Social Worker (LCSW), You Will Complete comprehensive psychosocial assessment with a focus on the patient’s requirements as they transition to the next level of care. Collaborate with physicians, nurses, and other disciplines involved in the patient’s care to foster a coordinated approach to discharge planning. Identify and navigate patient testing and treatment to reduce barriers to discharge and prevent delays in patient care; communicate barriers to leadership for resolution and trending. Communicate with Utilization Review staff on any denials, issues, or barriers to discharge. Identify services and resources available in the community and assist with patient connection to these services. Participate in Interdisciplinary Rounds and other patient care conferences. Document assessments and interventions according to departmental standards. Participate in process improvement activities. Qualifications Include Bachelor of Social Work (BSW) or Master of Social Work (MSW) Minimum of 3 years of experience in a diverse clinical setting is required. Prior case management or discharge planning experience is preferred. Prior experience with Cerner EMR is preferred. LBSW, LMSW, or LCSW Active American Heart Association Basic Life Support (BLS) certification required. Work Schedule Employment Type: Variable Part-Time Hours per Week/Month: M-F (24 hrs/month). Three 8‑hour shifts per month or six 4‑hour shifts per month. Shift Type: Days Weekend Requirements: None Holiday Requirements: Follow company holiday calendar. Shift Differential Eligible: Yes Pay Range $34.58 - $51.87 per hour. If the salary range is not displayed or if the position is Per Diem, salary discussions will take place during the screening process.
