Care Coordinator, Embedded (Waco and Temple, TX)

Overview Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in multiple states. Clinicians go to the home of the patient, providing continuity of care for those with chronic conditions and limited mobility. The Embedded Care Coordinator works on-site within one or more partnered long-term care facilities in the Waco and Temple, Texas areas, as part of a family-practice–style care team that emphasizes continuity, relationship-building, and patient-centered support. The role focuses on remote patient monitoring (RPM) to support enrollment and compliance, coordinate follow-up care, address barriers to care, communicate patient needs, and maintain accurate, timely documentation within the EMR. This is not a remote or call-center role. You are physically on-site in the facility, building relationships with residents and families and supporting care as a traditional family practice would, but in the long-term care setting. Key Responsibilities Legally authorized to work in the United States Active, unrestricted clinical certification or licensure in the state of Texas, such as Medical Assistant (MA) / Certified Medical Assistant (CMA) or an equivalent credential Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting Proactive, professional communication throughout the day via Microsoft Teams, email, phone, and in person Build and maintain positive, approachable relationships with residents, families, providers, and facility staff, with a consistent, dependable presence in the long-term care setting Strong organizational and time-management skills with an initiative-driven mindset Knowledge of healthcare terminology, diagnoses, medications, and care coordination principles Proficiency with Microsoft 365 (Outlook, Word, Excel) Full-time, Monday through Friday, 8:30 a.m. – 5:00 p.m. CST, on-site in the Waco/Temple, TX area No weekends, evenings, nights, or on-call requirements Daytime hours with seven company-paid holidays annually Transportation requirements: valid driver’s license, reliable transportation, current auto insurance with proof, and potential travel between facilities Must successfully complete a background check and drug screen Qualifications Minimum of Medical Assistant (MA) certification with at least one year of clinical experience Compassionate, detail-oriented professional suited to a fast-paced environment Strong organizational skills, adaptability, and clear, professional communication Benefits Full-time benefits eligibility with options such as Medical, Dental, Vision, Life Insurance, Disability, Flexible Spending Accounts, and other plans Paid Time Off Paid Office Holidays Paid Sick Time 401(k) with up to 3% company match Employee referrals, and pay-on-demand options Candidate disclosures: Ennoble Care is an Equal Opportunity Employer and does not discriminate against protected characteristics including race, age, sexual orientation, gender identity and expression, national origin, religion, disability, and veteran status. #J-18808-Ljbffr
