Hospice Nursing Case ManagerRN case manager needed for hospice program. The average daily census is 150 patients. Visits are made to patients in both private homes and facilities. The RN is scheduled with five points per day. Revisits count as one point and admits count as 2.75 points. Visit types and their corresponding point values include the following:Death at home: 0.5 pointsDischarge without visit: 0.5 pointsDiscipline discharge: 1 pointFollow-up: 2 pointsInitial evaluation: 2 pointsLab draw: 0.5 pointsOasis discharge: 1.5 pointsPreceptor day: 6 pointsRecertification: 2 pointsResumption of care: 2 pointsStart of care: 2.75 pointsFloating may be requested based on the needs of the facility. All units require specialty experience within the last year. Candidates must provide proof of certifications and licenses upon application. Prior employees must be gone for at least six months before being hired for travel roles. RTO must be kept to a minimum.
