Contract | 13-Week Assignment | Skilled Nursing Facility (SNF)

Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
Provide direct occupational therapy services to patients to restore, maintain, or improve functional independence
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities

Required Qualifications

Graduate of an accredited Occupational Therapy program
Current and valid Occupational Therapist licensure in the state of Wyoming or ability to obtain prior to start
National Board for Certification in Occupational Therapy (NBCOT) certification
Experience providing occupational therapy services in a clinical or post-acute care setting

Preferred Experience / Skills

Previous experience working in a skilled nursing facility (SNF) environment
Familiarity with Medicare documentation and compliance standards in a SNF setting
