Position Summary This position provides direct professional nursing care to patients through the implementation of the treatment plan. Initiates nursing standards, implements physical & psychological patient care, & evaluates responses to the care as a member of the interdisciplinary team. Major Duties and Responsibilities
CLINICAL JUDGEMENT:
Demonstrates a knowledge base necessary to care for adolescents through geriatric patients in the psychiatric care setting. Develops a treatment plan appropriate for the patient based on the nursing assessment, incorporating the family in the plan, and in collaboration with the interdisciplinary team. Recognizes the need to change the plan based on the patient’s strengths, goals and responses; and documents progress or lack of progress. Functions independently to arrive at an appropriate solution as a result of sound decision-making and critical thinking skills. Demonstrates the ability to prioritize patient care based on assessed needs and staff competencies. Seeks input and feedback when needed. Assumes responsibility and accountability for individual nursing judgments and actions. Demonstrates ability to assess the patient for risk of aggressive/escalating behavior. Engages patient who are at risk and applies alternatives to physical intervention by using verbal techniques to de‑escalate. Demonstrates appropriate use of physical intervention. Administers medication in accordance with standards and policy.ADVOCACY/MORAL AGENCY:
Demonstrates knowledge of and participates in departmental and organizational performance improvement. Identifies processes in the system and the therapeutic community that can be improved and provides this input to the appropriate individual. Maintains integrity and confidentiality of information. Attends treatment care conferences to assist with the discussion and planning of patient care. Supports the philosophy of patient and family centered care, recognizing the individual needs of the family. Supports family involvement in the care of the patient and supports family presence. Identifies issues of customer service and acts to resolve issues within available options. Refers complex and unresolved issues to the appropriate party.3. CARING PRACTICES:
Strives to meet and anticipate customer needs. Demonstrates flexibility in adapting to new, different, or changing requirements. Participates in and follows all scheduling guidelines, taking into account staffing needs of the unit to ensure delivery of high quality patient care. Collaborates with patient, family and interdisciplinary team in the development of the treatment plan and demonstrates the ability to communicate this plan to others via report or patient rounds. Demonstrates quality patient care and the positive mentoring of peers. Delegates appropriately to other members of the health care team. Demonstrates cost‑effective use of supplies and equipment.5. SYSTEMS THINKING:
Assists with the development and interpretation of departmental and institutional policies and procedures. Demonstrates routine use of the LCOH information system to read and respond to mail. Demonstrates the ability to use the EMR system to obtain lab values. Demonstrates adherence to policies and procedures during daily practice. Demonstrates the ability to use the online educational system to complete all mandatory education requirements. Completes unit specific competencies in a timely manner as required. Complies with all regulatory agency standards. Ensures proper reporting of unusual occurrences. RESPONSE to DIVERSITY: Implements treatments/procedures appropriate for patient utilizing age specific principles, while assessing patient’s comfort and well being. Demonstrates the ability to recognize cultural differences in the patient population. Utilizes the resources available to educate self to the different cultures encountered, and then demonstrates the ability to work with the family/patient to incorporate their differences and beliefs into the care they receive. Provides services with respect for human dignity and uniqueness, unrestricted by considerations of social or economic status, personal attributes or the nature of the health problems.CLINICAL INQUIRY:
Incorporates professional practice standards into clinical practice. Participates in the interdisciplinary and organizational wide performance improvement measures. Implement actions that result in continues improvement of quality and compliance. Participates in self‑evaluation and identifies personal learning needs in order to acquire and maintain current knowledge in behavioral health care nursing practice. Contributes to the professional development of peers, colleagues, and others. Assists with the development and mentoring of nursing students and new employees. Assures that an ethical framework has been used when making decisions for or acting on behalf of patients Implement changes in practice based on evidence base. FACILITATOR of LEARNING: Utilizes educational strategies including collaboration with hospital and community resources that are based on the needs of the patient and family. Begins the discharge process upon admission and accurately documents all teaching and discharge instructions on the appropriate form in a timely manner. Evaluates the need for patient/family education based on the nursing assessment and treatment plan. Provides thorough explanation of the treatment plan to the patient/family and assess the comprehension of information. Involves the patient and/or family in creating a safer patient care delivery process by encouraging them to ask questions about medications, progress toward treatment goals and outpatient follow‑up. Collaborates with referral sources and external agencies in the transition to outpatient follow‑up. Position Qualifications Registered Nurse with licensure in the State of Ohio. Graduate of an accredited school of nursing. CPR Certification required within 30 days of hire. Certification of Psychiatric Mental Health Nursing from the American Nurse Association Credentialing Center (preferred) Demonstrates the knowledge and skillsnecessary to provide age‑specific and developmentally appropriate care for the spectrum of populations admitted to LCOH. Effective leadership skills and the ability to build a strong departmental team. Function effectively as a team member with other staff members of the Lindner Center of HOPE. Ability to enthusiastically follow and model the Lindner Center of HOPE mission, vision and values. Physical Requirements To effectively care for patients, candidates must be able to push, pull and lift up to 50 lbs on a regular basis, and be able to stand or walk about 90% of a work shift.
