Discharge Lounge Nurse

TriStar Health is seeking a Discharge Lounge Nurse to support safe, efficient patient transitions home within our Healthcare & Medical Services department. As an RN, you will assess and monitor patients awaiting discharge, administer medications, coordinate discharge plans, and provide thorough education to patients and families. You’ll collaborate closely with physicians, case management, and multidisciplinary teams while documenting care accurately in the EHR.
