LHH is teaming up with a mission‑driven nonprofit organization in Sacramento, CA in search of a Case Worker to support individuals and families through resource coordination, case management, and ongoing support services.

The Case Worker will play a key role in managing client relationships, maintaining accurate documentation, and ensuring each case progresses efficiently. The ideal candidate is someone who is highly organized, compassionate, and comfortable managing multiple client touchpoints while maintaining strong documentation standards. They bring at least one year of case management or related experience, along with strong communication skills and the ability to build trust with diverse populations. Bilingual capabilities in Farsi, Dari, or Pashto are required. This individual should be reliable, detail-oriented, and able to balance administrative responsibilities with client-centered care.
Key Responsibilities:
- Maintain accurate, up‑to‑date case notes and client files with a strong emphasis on compliance
- Schedule, attend, and document client meetings and follow-ups
- Connect clients with appropriate community resources and service providers
- Manage 5–7 active cases at a time, with case durations typically ranging from 30–90 days
- Complete documentation-heavy administrative tasks and ensure timely data entry
- Track and maintain case data using Microsoft Excel and internal systems
- Ensure confidentiality and handle sensitive client information with discretion
- Provide ongoing support to clients, including frequent communication and check-ins
Qualifications:
- Minimum 1 year of case management or related experience (required)
- Strong organizational, documentation, and administrative skills (required)
- Ability to manage multiple client interactions and maintain professionalism (required)
- Bilingual skills in Farsi, Dari, or Pashto (required)
- Experience working in nonprofit or community-based settings (preferred)
- Familiarity with Sacramento community resources (preferred)
- Strong interpersonal skills with an empathetic, non-judgmental approach (required)
- Highly detail-oriented, dependable, and resourceful (required)
Job Type: Contract (estimated duration through September 2026)
Start Date: ASAP
Location: Fully on-site in Sacramento, CA
Hours: Monday–Friday, 9:00 AM–5:30 PM
Pay Rate: $22–26/hour, depending on experience
