Oak Medical is seeking a Nurse Practitioner or Physician Assistant to join the telehealth team serving Skilled Nursing and Assisted Living facilities in Wisconsin. You will conduct telehealth visits, manage chronic conditions, and coordinate care with facility staff and primary care providers.
