Salary range: $58.00 – $81.20 (Hourly Rate) Benefits 10,000 USD Tuition Reimbursement per year (5,700 USD part‑time) 10,000 USD Student Loan Repayment (5,000 USD part‑time) 1,000 USD Professional Development per year (500 USD part‑time) 250 USD Wellbeing Fund per year (125 USD for part‑time) Matching 401(k) Excellent medical, dental and vision coverage Life insurance Annual employee salary increase and incentive bonus Paid time off and holiday pay Job Overview The Clinical Pharmacist is accountable for identifying, preventing, and resolving medication‑related problems; providing authoritative information to healthcare professionals, patients, and the public; and ensuring the pharmacy services operate as an integrated system to deliver patient‑centered care at Northwestern Medicine. Responsibilities Interpret medication orders to ensure safety and avoid incompatibilities, duplications, allergies, and potential administration errors, coordinating solutions with pharmacy, nursing, and prescribers. Monitor medication therapy for significant interactions, intervene when necessary, and apply pharmacokinetic and pharmacodynamic expertise. Evaluate and document medication appropriateness, communicating problems and solutions to prescribers. Act as a consultant on pharmacokinetics and adverse reactions, participating in therapeutic drug monitoring and adjusting therapy per patient condition and lab results. Process medication order information into the Electronic Health Record, ensuring accurate dispensing and charging records. Maintain comprehensive patient pharmacy profiles including age, weight, allergies, diagnosis, and relevant pharmacokinetic data. Oversee packaging, labeling, and distribution of medications for patients and employees, monitor scheduled medication use, and maintain controlled inventories. Compound and dispense pharmaceuticals including sterile products, chemotherapy, and parenteral nutrition. Verify medication orders before delivery, ensuring accuracy and resolving discrepancies. Review laboratory data for medication appropriateness and recommend changes, including culture, sensitivity, toxicology, and therapeutic monitoring. Develop and present orientation, education, and training programs for medical, nursing, hospital, and pharmacy staff, and assist in educating pharmacy students, residents, and fellows. Educate patients and families about specific medications. Respond to emergency situations involving medication management. Provide accurate, timely medication information and teaching to healthcare professionals or patients. Maintain the hospital formulary and limit usage to formulary items when appropriate. Document all clinical activities and interventions; collect data and maintain records as required. Communicate professional knowledge effectively in oral and written forms. Participate in quality improvement and medication use review activities. Organize, prioritize, and plan workload within time constraints; work cooperatively with pharmacists to oversee work area activities and assign technician work. Review ADC restocking and resolve any problems. Ensure compliance with hazardous waste collection and disposal procedures. Maintain proficiency in areas of practice through continuous professional development. Perform all other assigned duties. Qualifications Required: Active Clinical Pharmacist license in the state of Illinois (IDFPR). Bachelor’s degree in Pharmacy or a Doctor of Pharmacy degree. Preferred: Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.). Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
