Clinical Social Worker, Inpatient Psychiatry Per Diem

The per diem social worker works collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community, and assesses and arranges discharge planning needs and post hospital follow up disposition.Responsibilities include:Individual psychiatric evaluationsDaily individual contact with assigned patientsLeading one or more weekly social work treatment groups (as needed)Family outreach and counseling and psychoeducation as indicatedCoordination with outpatient treatment teamAssesses and arranges discharge planning needs and follow up as appropriateTimely completion of Treatment Plan and Discharge Summary that will satisfy clinical and regulatory requirements and arranging referrals related to aftercare as neededCollaborates with and provides social work consultation to other disciplines within the settingParticipates on Departmental and Hospital committees as appropriateOther duties as assignedPer diem as needed including weekdays, weekends, vacation coverage. Minimum 24 hours per month.Qualifications:Master's of Social Work (MSW) from an accredited program required.Massachusetts Licensed Independent Clinical Social Worker (LICSW) OR Licensed Clinical Social Worker (LCSW) with relevant clinical experience required.Minimum of graduate level internship experience in relevant setting.Staff adheres to all C.A.R.E. StandardsKnowledge of specific medical/psychiatric illnesses, procedures and treatmentsExcellent interpersonal skills including negotiation skills necessary to work within a teamAbility to provide rapid clinical psychosocial and psychiatric assessments and brief, short or long term treatment/management with individuals, families, couples and/or groups, and assess discharge planning needs.Cultural sensitivity and demonstrated competency in age specific behaviorsAbility to work effectively as a member of a multi-disciplinary teamKnowledge of community agencies/resources.
