Cook County Health & Hospitals System in Chicago seeks a Senior Manager of Complex Care Coordination, Community Care, to provide leadership across complex care coordination initiatives, oversee analytics, SLAs, grievances, project implementation and daily operations, and to coordinate with Health Plan partners. The role requires a Master’s degree, licensure as LSW/LCSW or RN, and several years in Medicaid Managed Care, data analysis and staff supervision; strong MS Office skills are essential. #J-18808-Ljbffr Direct Jobs
