Position SummaryUnder the supervision of the Director of Social Services, and in consultation with the Assistant Director of Social Services, the Clinical Case Manager provides comprehensive case management services to individuals with complex medical, behavioral health, and psychosocial needs. This position works collaboratively with multidisciplinary teams to help clients overcome barriers to care, improve treatment adherence, access community resources, and enhance overall quality of life. The Clinical Case Manager serves as an advocate, care coordinator, counselor, and resource for vulnerable and at-risk populations.
Key Responsibilities- Conduct outreach to educate clients about available social services and increase engagement in medical and behavioral health care.
- Utilize person-centered, strength-based, and motivational interviewing techniques to build therapeutic relationships with clients.
- Assess clients' psychosocial, behavioral health, and social service needs, including housing instability, HIV/AIDS, substance use, mental health, domestic violence, pregnancy, and other social determinants of health.
- Develop and maintain individualized, person-centered case management plans.
- Assist clients in accessing healthcare, behavioral health treatment, insurance benefits, and community resources.
- Provide advocacy, referrals, crisis intervention, and ongoing case management support.
- Coordinate care with medical providers, behavioral health clinicians, and interdisciplinary teams.
- Complete psychosocial assessments, mental status examinations (as appropriate based on licensure), and medical summary reports.
- Provide brief counseling, solution-focused therapy, psychoeducational groups, and behavioral health support within scope of practice.
- Maintain accurate, timely documentation in the electronic health record and other required systems.
- Participate in case conferences, staff training, continuing education, and departmental meetings.
- Maintain professional licensure, ethical standards, and appropriate professional boundaries.
- Perform other duties as assigned.
- Master's degree in Social Work, Psychology, Counseling, or another Human Services-related field.
- Current District of Columbia licensure as one of the following:
- Licensed Graduate Social Worker (LGSW) or Licensed Independent Clinical Social Worker (LICSW), or
- Licensed Graduate Professional Counselor (LGPC) or Licensed Professional Counselor (LPC).
- Minimum of two (2) years of post-master's professional experience.
- Minimum of four (4) years of experience working in community health, behavioral health, or social service settings.
- Case management experience required.
- Experience working with vulnerable, underserved, and at-risk populations.
- Ability to obtain SOAR Certification within 90 days of hire.
- Strong knowledge of behavioral health, DSM diagnoses, community resources, and social service systems.
- Excellent communication, documentation, organizational, and interpersonal skills.
- Proficiency with electronic health records and Microsoft Office 365 applications.
- Ability to work independently while collaborating effectively within a multidisciplinary healthcare team.
