RN Case Manager: Care Coordination & Discharge Planning

UMass Memorial Health is seeking a Case Manager to coordinate and facilitate the management of care for an assigned group of patients. The role emphasizes early discharge planning, collaboration with the healthcare team, and ensuring a well-coordinated, cost-conscious care experience.

Ideal candidates hold a BSN and Massachusetts RN license, with 3–5 years of clinical experience and strong interpersonal/analytical skills. A CCM certification is preferred and a signing bonus may be offered.
