We are looking for an Advanced Practice Nurse to join our established primary care team. This role provides the chance to deliver comprehensive primary care in a collaborative setting while establishing lasting connections with patients.

Duties

• Deliver primary care services in a primary care setting
• Evaluate, identify, and manage acute and chronic illnesses
• Create care plans and recommend medications when necessary
• Educate patients on preventative care and wellbeing
• Work closely with physicians and clinical staff to ensure high-quality patient care
• Maintain precise medical records

Requirements

• Completion of an accredited Advanced Practice Nurse program
• Active RN license in New York and certification as an Advanced Practice Nurse
• Current BLS certification (ACLS is a plus)
• Excellent clinical and communication abilities
• New graduates are welcome to apply

Compensation & Perks

• Salary range: $98,000 $143,000
