Responsibilities Medication & Treatments: Prepare and administer medications (including injections and prescribed treatments) and transcribe physician orders accurately. Clinical Monitoring: Monitor vital signs, track fluid intake/output, and observe patients for progress or medication side effects. Documentation: Maintain accurate patient charts, medical histories, and daily observations. Direct Care: Assist patients with daily activities, including wound care, dressing, and hygiene. Support & Communication: Provide emotional support to patients and communicate effectively with family members regarding care instructions. Collaboration: Perform additional duties as assigned by the Director of Nursing to ensure high-quality care. Qualifications License: Must hold an active and unrestricted Licensed Practical Nurse (LPN) license in the state of Illinois. Experience: Proven experience in providing diverse basic care; long-term care experience preferred. Soft Skills: Compassionate, patient, and able to remain calm in a demanding environment.
