Check out our website: Home - FortisJob SummarySelect Home Health is seeking skilled and compassionate Registered Nurse to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.Duties/Responsibilities:Perform patient assessments and ongoing evaluations.Implement physician-ordered plans of care and complete required documentation (OASIS).Provide in-home nursing care for multiple patients per day.Educate patients and caregivers on medications, disease processes, and in-home care.Coordinate patient services and supervise LPNs/HHAs as needed.Communicate changes in patient condition to physicians and care teams.Maintain compliance with regulations and agency policies.Physical RequirementsAbility to sit, stand, and drive for extended periods while providing in-home care.Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.Regular use of a tablet for documentation, communication, and training.Disclaimer:This description reflects the general nature and level of work for the Registered Nurse Position.
