About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty. License: Must have a valid nursing license. Certifications: CPR, BLS Must-Have: Must have recent specialty experience. Proficiency in HomeCare HomeBase EMR and OASIS used within the last 6 months. Must be willing to travel up to 75 miles from the assigned care center. Description: Provides coordinates and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures. Responsible for the competent application of the nursing process. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Responsibilities include home visits and participation in periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average case load is 5-10 patients per day, equating to roughly 6-8 points. Each visit and activity, including documentation and travel, hold different point values, with a total of 30 points expected per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Weekend requirements vary per location and must be discussed during the interview. Traveling responsibilities include commuting between patient homes and the clinic for weekly meetings. The contractor will accept every patient on their schedule regardless of distance from previous appointments. All documentation must be completed within 24 hours of the appointment. There are no guaranteed hours with this contract. The clinician must hit weekly productivity points of 30, equating to 40 hours per week. Floating to another nearby location may be available if patient census is low. Local candidates are accepted at the same rate.
