Additional Job Description: POSITION SUMMARY : The Advance Practice Professional (APP)is an individual with clinical skills and medical skills, with anexpertise in the care of Hematology/Oncology (hem/ onc ) pediatricpatients. The APP completes comprehensive patient healthevaluations (including biological, social and psychologicalassessments), taking into account their co-morbid conditions andoverall disposition; in collaboration with the appropriateclinical/faculty team. The APP assesses, and coordinates theeducational and psychosocial needs of the patient and caretakers.The APP develops and reviews the plan of care with the othermembers of the clinical team, participates in directing that care,and coordinates discharge planning/and or follow-up (asapplicable). The APP participates in the education of residentsand medical/nursing students and performs diagnostic procedures andtherapeutic procedures. Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable . Suchservice may include, but are not limited to . MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Graduate of a CCNE accredited nursing program with Master’sDegree in Nursing . 2. National Certification by one of the recognized APRNCertification Centers. 3. Licensure, or eligibility for licensure, as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia. 4. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion of thedepartment. 5. American Heart Association Advanced Cardiac Life Support (ACLS)at the discretion of the department. 6. American Heart Association Pediatric Advanced Life Support(PALS) at the discretion of the department . EXPERIENCE: 1. Eligible new graduates will be considered. PREFERRED QUALIFICATIONS : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. OCN or AOCN certification preferred EXPERIENCE: 1. Hematology/oncology, acute care or pediatric experiencepreferred . MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor’s Degree or Master’s Degree as Physician Assistantrequired . 2. Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required . 3. Certification by the National Commission for Certification ofPhysician Assistants required . 4. Must be licensed or eligible for licensure by the WV Board ofMedicine or Board of Osteopathic Medicine . 5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion of thedepartment . 6. American Heart Association Advanced Cardiac Life Support (ACLS)at the discretion of the department. 7. American Heart Association Pediatric Advanced Life Support(PALS) at the discretion of the department . EXPERIENCE: 1. Eligible new graduates will be considered. Hematology/oncology, acute care or pediatric experiencepreferred . CORE DUTIES AND RESPONSIBILITIES: The statements describedhere are intended to describe the general nature of work beingperformed by people assigned to this position . They are notintended to be constructed as an all-inclusive list of allresponsibilities and duties . Other duties may be assigned. 1. Manages comprehensive and individualized care for Endocrine, GI,Immunology, Rheumatology and Nephrology patients and their familiesin the infusion center, during inpatient hospitalization and in theambulatory setting. 2. Completes daily rounds; consults and/or pre-treatment orpre-procedure/evaluations in the infusion center . Participates inambulatory clinics as applicable. 3. Completes integrated assessments. 4. With the collaboration of hem/ onc faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate . 5. Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice. 6. Consults and communicates with clinic and hospital staffregarding abnormal findings in the evaluations of patients. 7. Performs succinct comprehensive clinical evaluations anddocuments findings in medical record . 8. Facilitates patient discharge and/or follow-up byordering/coordinating appropriate services and tests prior todischarge from the infusion center necessary for outpatientfollow-up appointments in the clinic (as applicable) . 9. Performs necessary procedures following established clinicalguidelines and Protocols and scope of practice (SOP). 10. Develops a comprehensive individualized plan of care for hem/onc pediatric patients, which includes acute management, teaching,discharge planning and necessary referral to other members of thehealth care team as necessary. 11. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification. 12 Participate in physician rounds, ( as applicable). 13. Identifies unstable or complex hem/ onc pediatric patient caresituations and facilitates appropriate intervention in the infusioncenter, clinic and/or hospital setting. 14. Recognizes emergency situations and initiates effectiveemergency care. 15. Evaluates hem/ onc pediatric patients and directs follow upprocedures and tests in the ambulatory setting (asapplicable). 16. Assists in the collection of outcome measures (asapplicable). 17. Demonstrates critical thinking and diagnostic reasoning skillsin clinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families. 18. Demonstrates the ability to utilize equipment in a safemanner. 19. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings. 20. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community. 21. Promotes an environment that facilitates learning by respondingto appropriate patient/family preferences, priorities and readinessto learn. 22. Evaluates the effectiveness of patient/family teaching andcollaborates with the nursing and clinical staff in the completionof education plan . 23. Updates own knowledge base throughout the year. 24. Completes all identified clinical competencies. 25. Serves as a resource person for staff caring for the hem/ oncpediatric patient. 26. Provides educational programs to professional andparaprofessional personnel in area of expertise. 27. Mentors nursing personnel in all levels of clinical practice. 28. Participates in community programs, distribution of literature,as appropriate in area of expertise. 29. Communicates effectively with patients/families, physician andancillary services at all levels to assure coordinated efficientpatient care . 30. Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation . 31. Documents all interventions in the patient’s medical recordfollowing approved hospital/UHA policies and compliance standardsfor documentation . 32. Collaborates with other leaders and staff to effectivelyachieve patient outcome based improvements in care. 33. Provides leadership in the development and implantation ofchanges in the hem/ onc practice that positively impact patientoutcomes . 34. Works collaboratively with medical and nursing staff tofacilitate creative problem solving and provide professionalsupport. 35. Participates and initiates inter-disciplinary approaches topatient care, program development and education . Serves as aprofessional role model. 36. Promotes and practices innovation of the expanded role in thedelivery of care to hem/ onc pediatric patients and families in theinfusion center, the inpatient hospitalization and the ambulatorysetting. 37. Participates and provides leadership for committees and taskforces. 38. Documents hours and submits billing reports, as appropriate. 39. Participates in the WVUH/UHA Performance Improvement Plan. 40. Applies information from Performance Improvement activities,changing practice as necessary, to improve care provided topatients . 41. Reviews and interprets hem/ onc data from sources such aspatient registries, clinical and financial reports, length of stay,access to care, and cost per case reports and recommends/implementschanges as necessary. 42. Participates in clinical trials and research studies with thePrincipal Investigator. 43. Assists Faculty, Administrators and Directors in the collectionof outcome measures. 44. Adheres to the established Performance Expectations for UHAEmployees in the areas of People, Service, Performance Improvement,and Shared Values & Culture. 45. Oversees and provides comprehensive and individualized care forevaluating and managing patients in the infusion center withinpractice scope. 46. Provides direct supervision of patients in the infusion centeruntil last patient receiving an infusion leaves the infusioncenter.
