Key Responsibilities

• Provide occupational therapy evaluations and treatments to home health patients across an assigned caseload in the Concord and Walnut Creek area
• Actively manage caseload to maintain expected productivity, targeting a minimum of 30 points per week
• Proactively backfill cancelled visits by contacting patients to reschedule or advance upcoming appointments
• Complete clinical documentation accurately and in a timely manner using HomeCare HomeBase (HCHB)
• Collaborate with a supportive home office team to coordinate patient care and scheduling

Required Qualifications

• Active Occupational Therapist (OT) license in the state of California
• Minimum 1–2 years of occupational therapy experience
• Proficiency or willingness to work with HomeCare HomeBase (HCHB) documentation platform
• Valid driver's license and reliable transportation for home visit travel

Preferred Experience / Skills

• Prior home health occupational therapy experience
• Existing familiarity with HCHB electronic documentation
• Strong self-management skills and ability to independently manage a dynamic caseload
