LPN Case ManagerWe are seeking an LPN Case Manager to join our team in our Crafton, PA location.Earn $29 per hour with benefits or $31 per hour with the selection of enhanced pay and scaled back benefits.Schedule: M - F 8:30 AM - 9:00 PMPosition DetailsResponsibility and accountability for the overall coordination of the medical portion of the plan of care and services for a designated caseload of residentsUtilization of a collaborative approach by the NCM with all members of the health care team will ensure the plan implementationNCMs will function as the nursing liaison across disciplines within the AVS system, with family members/caregivers, and with community-based health care providers/agencies to ensure medical and nursing care is appropriate and timely.Program DetailsMerakey AVS is a non-profit organization which provides homes and a full range of services to adults with all levels of intellectual and developmental disabilities (IDD).
