RN Case Manager: Discharge & Care Coordination

Ochsner Health

Ochsner Health in Louisiana seeks a Registered Nurse for discharge planning and utilization review. You will perform pre-admission, admission, and continued stay reviews, coordinating with ED physicians and hospitalists to ensure medically necessary care and proper discharge planning. You will liaise with patients, caregivers and multidisciplinary teams, maintaining compliance with CMS guidelines and hospital policy. #J-18808-Ljbffr
