Clinical Intake Specialist (Home Health & Hospice)

Primary responsibilities will be:

• Providing an outstanding admissions experience for our patients
• Communicating effectively with patients, family members, case managers, healthcare providers, and co-workers
• Providing education to patients and family members regarding their home healthcare experience
• Reviewing patient charts and ensuring information is clearly communicated to the patients care team
• Serving as a point of contact throughout the admissions process
• Ensuring coordination of care with other team members

Must-haves to be considered for this position:

• 1 year of intake experience (home health/hospice)
• Ability to multi-task with high volume referrals
• Insurance verification and multi hospital portals (experience is a must)
• Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• LVN is a plus but not mandatory
• Experience with HCHB EMR
