RN Care Manager

Spectrum Health

Responsibilities Plans and prioritizes care for individuals and populations, focusing on strategies that promote optimal health. Demonstrates expertise in nursing care and management of a caseload of patients of varying complexity, seeking to improve patient, family and community outcomes through educational concepts and preventive care in a managed care environment. Assesses internal and external referrals to identify patient and significant others’ needs, intensity levels, insurance benefits, and other resources. Develops plans of care and makes recommendations to PCPs, specialists and other health‑care team members regarding care‑management strategies, maximizing continuity of care across the continuum. Assesses educational needs of patients, families and team members and develops/implements appropriate teaching strategies or makes referrals. Communicates and collaborates with patients/significant others, providers and payers to coordinate services that improve access to appropriate services across the continuum and promote optimal health cost‑effectively. Measures patient‑care outcomes, interprets reports and analyzes data trends for groups of patients. Documents patient data, plans, interventions and outcomes in accordance with departmental guidelines. Qualifications Bachelor’s Degree or equivalent in Nursing or a health‑care related field (BSN). The BSN must be obtained within seven years of the start date for RN roles on or after January1,2017 (Corewell Health). Five years of relevant experience in clinical nursing and/or case management/managed‑care or related field (required). At least two years of relevant experience is preferred. Registered Nurse (RN) license in the State of Michigan (required). Certified Case Manager (CCM) – CCMC Commission for Case Manager Certification (preferred).
