Health/Human Service Case Manager II (Bilingual Case Manager)

The Health & Human Services Case Manager II is responsible for providing comprehensive case management services to individuals and families, including conducting assessments, developing service plans, coordinating resources, and ensuring timely access to essential supports. This position maintains accurate documentation, meets regulatory requirements, and collaborates with internal teams and community partners to promote client stability and well-being. Because a significant portion of residents primarily speak Spanish, the Case Manager II provides bilingual communication to ensure equitable access to services, reduce language barriers, and support clear, compassionate interactions between residents and staff.
