Advanced Mobile Crisis Case Manager | CPST Certified
Join a dedicated team providing intensive, goal-oriented support to individuals facing mental health crises and substance use challenges requiring crisis stabilization. This role involves close collaboration with clinical and community partners to deliver services with increased intensity and frequency, aiming to help consumers maintain community tenure.
Work Schedule
This full-time position operates Monday through Friday from 12 PM to 8:30 PM, with occasional night and weekend shifts based on community crisis demands and coordination with management.
Essential Responsibilities
- Deliver person-centered case management using established methods to alleviate mental illness symptoms and enhance emotional and functional well-being.
- Provide crisis stabilization services to individuals experiencing mental health emergencies in the community or Crisis Stabilization Unit.
- Conduct trauma-informed risk assessments under the guidance of a Qualified Mental Health Professional to address safety concerns.
- Apply de-escalation techniques effectively in high-intensity situations.
- Collaborate regularly with community partners, caregivers, family members, physicians, and other treatment providers until crisis resolution.
- Support consumers in achieving treatment goals, coordinate with collateral contacts, provide medication education, and assist with crisis management.
- Encourage clients to complete daily living activities and monitor safety as needed.
- Participate in care coordination to arrange and monitor services, ensuring clients attend mental health appointments.
- Work towards successful reintegration of consumers into their homes, communities, schools, and outpatient services.
- Complete all required documentation accurately and promptly in accordance with agency standards.
- Adhere to privacy and security regulations regarding Protected Health Information (PHI).
- Maintain professional communication and relationships with clients, team members, contractors, physicians, peers, outside agencies, and the public.
Education and Experience
- Bachelor's degree preferred in Social Work, Psychology, or related fields.
- Associate's degree with two or more years of experience educating and teaching clients about self-care and improving daily living activities is acceptable.
- Total combined education and experience should equal four or more years.
Licenses and Certifications
- CPST Case Management training and KHS training required.
- Pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screenings.
- Completion of Adult Case Management training within 60 days of employment.
- Valid driver's license, insurance, and reliable vehicle if driving is required.
Preferred Qualifications
- Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
- Three to five years of professional experience in mental health services, crisis response, or related fields.
- Two or more years of experience educating and teaching clients about self-care and daily living improvements.
Skills and Abilities
- Familiarity with community mental health services.
- Strong organizational skills.
- Intermediate proficiency with Microsoft Office.
Physical Requirements
Medium work involving occasional exertion up to 50 lbs and frequent exertion up to 10 lbs. Approximately 26-50% of the day may involve standing or walking. Accommodations available as needed.
