Geriatric Primary Care Physician (MD/DO) Medicare Advantage
ClareMedica Health Partners
Claremedica seeks a Board-certified/eligible Physician (Family/General/Internal Medicine) to deliver high-quality primary care to seniors, with focus on Medicare Advantage, preventive care, and chronic disease management.
You will work in a team-based environment, coordinating care with case managers and risk adjustment specialists, while maintaining EMR documentation and adherence to quality targets. Florida license and 2+ years' experience preferred.
