About the Role

As a Registered Nurse on our team, you will be responsible for delivering high-quality, patient-centered care to our residents and patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and their families. Your expertise and dedication will play a critical role in ensuring the overall health, well-being, and satisfaction of our residents and patients.

Responsibilities

• Provide direct patient care, including assessing, planning, implementing, and evaluating care plans
• Conduct patient assessments, identify areas of concern, and develop and implement individualized care plans
• Collaborate with the interdisciplinary team to ensure comprehensive care and effective communication
• Educate patients, families, and caregivers on disease processes, treatment options, and self-care techniques
• Monitor patient progress, identify areas for improvement, and adjust care plans accordingly
• Participate in quality improvement initiatives and contribute to a culture of excellence
