Caregiver Support Program Clinical Assessor

The Caregiver Support Program (CSP) Clinical Assessor is responsible for knowing eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day-to-day operation of the Caregiver Support Program (CSP), and all reporting requirements within the assigned Veterans Integrated Systems Network (VISN). The CSP includes the PCAFC and the Program of General Caregiver Support Services (PGCSS) within the Veterans Health Administration (VHA). Duties include, but are not limited to:

• Assesses, plans, implements, and evaluates care based on age-specific components
• Responsible and accountable for all elements of the nursing process when providing direct patient care
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care
• Follows procedures per established policies and guidelines
• Influences care outcomes by collaborating with members of the interdisciplinary team
• Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care
• Conducts home visits to caregivers and Veterans approved in the program and provides assessments at Veterans Affairs (VA) facilities, via phone and telehealth
• Provides referrals and/or consults to VA and community based social services
• Collaborates with the interdisciplinary team for case review with high inter-rater reliability, efficiency, and objectivity as top priorities
• Applies principles of the CSP, CEAT practice, nursing process and care coordination principles within a collaborative, interdisciplinary practice setting
• Assesses patient care needs to determine the most appropriate level of program participation
• Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise, etc.

Pay: Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: TBD (Typically M-F, 0800-1630 CST)
